To start, Forest View Hospital will conduct a complete, confidential assessment, at no cost. The initial assessment is designed to determine the level of care that best meets the person’s needs.

What to Bring

• Appropriate, comfortable clothing, without drawstrings (Pack for at least three days. Additional clothing can be supplied if needed)
• Toiletries (additional toiletries can be supplied if needed)
• Important phone numbers
• Photo Identification
• Insurance cards
• List of providers/medications
• A book and/or a journal (no electronics allowed)
