What to Expect
To start, Forest View Hospital will conduct a complete, confidential assessment, at no cost. The initial assessment is designed to determine the level of care that best meets the person’s needs.
What to Bring
- Appropriate, comfortable clothing, without drawstrings (Pack for at least three days. Additional clothing can be supplied if needed)
- Toiletries (additional toiletries can be supplied if needed)
- Important phone numbers
- Photo Identification
- Insurance cards
- List of providers/medications
- A book and/or a journal (no electronics allowed)
